The bikini incision uses the same muscle-sparing anterior interval with a skin-crease scar that sits under swimwear. Same-day discharge is offered at UHealth when you qualify.
Patients hear “bikini hip replacement” and imagine a different operation. It is not. The deep interval is the same internervous, intermuscular plane used for a standard anterior approach — between the tensor fascia latae and the sartorius–rectus corridor. No major posterior or lateral stabilizers are detached.
The bikini (or “bikini-line”) incision is a skin-crease or oblique cut in the groin fold instead of a longitudinal cut down the front of the thigh. Hospital for Special Surgery and other high-volume anterior programs describe it as a cosmetic skin variation on the same anterior interval, not a new way to replace the socket and stem.
I still use AI fluoroscopic navigation to check cup position, offset, and leg length before we leave the room. The incision style does not replace that check.
Miami and South Florida patients often ask about swimwear, fitted clothing, and the gym. A crease incision can hide under a swimsuit or brief more readily than a vertical anterior scar. That is a real quality-of-life preference. It is not a promise of an invisible scar, and it is not a reason to choose a surgeon who does not otherwise fit your anatomy.
AAOS OrthoInfo explains that hip replacement is done to relieve arthritis pain and restore walking — not to guarantee a particular cosmetic result. I will tell you in clinic whether your skin crease, body habitus, and bone anatomy make a bikini incision a good idea.
Anterior hip replacement is muscle-sparing compared with approaches that detach the short external rotators or abductors. That is why many of my anterior-approach patients do not receive the old posterior list of hip precautions (no bending past 90 degrees, no crossing, no twisting). Your plan is still individualized.
Muscle-sparing is not painless surgery. You will have swelling. You will use a walker or cane at first. Same-day discharge at UHealth still requires you to walk, manage stairs, and have pain controlled on oral medicine.
The posterior approach remains a well-established option used by many excellent surgeons. This practice prefers anterior — including a bikini skin cut when it fits — for primary hip replacement in appropriate patients.
Many primary anterior hips go home the same day from UHealth HSS SoLé Mia or UHealth Tower when selection is right: stable medical history, adult support at home, and the ability to walk a meaningful distance after surgery. OrthoInfo and AAHKS both note that hospital stay has shortened as anesthesia, blood-loss control, and therapy pathways improved. They do not say everyone must go home the same day.
Typical pattern in healthy candidates:
Driving, work, and sport are individualized. I will not put a calendar date on “pain-free.”
Every hip replacement carries infection, blood-clot, fracture, dislocation, implant wear or loosening, and medical risk. Anterior-specific issues include lateral femoral cutaneous nerve irritation — a patch of thigh numbness or tingling. HSS patient education notes that this is usually bothersome rather than dangerous, and sensation often improves over months.
We lower risk with medical optimization, antibiotics, clot prevention, early walking, and intraoperative imaging. Dislocation risk is lower with modern anterior technique and careful cup position. It is not zero.
Good bikini-incision candidates are usually the same patients who are good anterior-hip candidates: symptomatic hip arthritis, failed non-operative care, and anatomy that allows a safe front-of-hip interval. Very high BMI, complex revision, or unusual bone deformity may push us to a different skin cut or a different approach.
Q: Is bikini-incision hip replacement less invasive than regular anterior hip replacement? A: The deep muscle interval is the same. The skin cut is different. Recovery is driven by the joint work, not by the word “bikini.”
Q: Will I have thigh numbness? A: You might. The lateral femoral cutaneous nerve sits near the interval. Most cases improve. Tell us if numbness is spreading or painful rather than a stable patch.
Q: Can I go home the same day? A: Many patients do, at UHealth, if they meet discharge criteria. If you do not, you stay. That is appropriate care, not a failure.
Q: Does the bikini incision change implant longevity? A: No. Implant survivorship depends on bone, bearing, activity, and position — not on whether the skin cut was creased or longitudinal. Modern replacements are designed as long-term solutions. Large registries report high survivorship into the second decade for many patients, and newer designs are hoped to last longer. Individual results vary. No implant is guaranteed for a set number of years.
If hip pain is limiting walking, swimming, or work, call 305-243-3000 or request a visit at miamijointreplacement.com/book. We will look at your X-rays the same day when possible and decide whether anterior hip replacement — bikini incision or longitudinal — is the right operation for you.
Educational information from the practice of Steven Denyer, MD, MS. It is not a diagnosis, treatment plan, or a promise of results. Joint replacement is surgery and carries risks. Suitability, recovery, and implant longevity vary. Discuss your own health, imaging, and goals with a qualified orthopedic surgeon before making a decision.
Educational information only. Individual candidacy, outcomes, and recovery timelines vary and require medical evaluation.
